  • All members must complete and sign the below application

  • $100.00 check per family membership fee

  • Contact LittleT Secretary for instructions on how to pay online using Square.

  • Board members must be LTQMC members prior to 1 January 2025 and NYS (USAC) members within 30 days of registration opening.

  • For new members, include a copy of the driver(s) birth certificate(s)
